As of 8/19/2026 we are contacting (via email) those who filled out this form in December 2025.

If you completed this form prior to December 2025, please check your junk/spam folder, and email if you do not see a message from us regarding scheduling. We do send emails securely, so it may say you’ve received a “hushmail” message, which is our secure email service.

Once you are contacted for scheduling a new patient appointment, we are generally scheduling out about 1-3 months. Example: those contacted in August are scheduling appointments in September-November.
