If you are interested in becoming a patient, please note that we estimate it to be about 12 months from the time one fills out this form, to when they have an appointment with Tasha. We update this page monthly with where we are regarding scheduling timeline. Filling out this form adds you to the waitlist. You will get an email confirmation, but will not receive any other correspondence from us until it is your turn to schedule.

As of 9/23/2026 we are contacting (via email) those who filled out this form in January 2026.

If you completed this form prior to January 2026, please check your junk/spam folder, and email if you do not see a message from us regarding scheduling. We do send emails securely, so it may say you’ve received a “hushmail” message, which is our secure email service.

Once you are contacted for scheduling a new patient appointment, we are generally scheduling out about 1-3 months. Example: those contacted in August are scheduling appointments in September-November.
