If you are interested in becoming a patient, we are currently scheduling out at least 9 months, so it may be awhile before you are contacted. If you previously filled out this form we are actively reaching out to folks for scheduling in the order received.

As of 6/17/2026 we are working to contact those added to the waitlist in December 2025. If you completed this form prior to December 2025, please check your junk/ spam folder, and email if you do not see a message from us regarding scheduling.

Once you are contacted for scheduling a new patient appointment, we are generally scheduling out about 3 months. Example: those contacted in June are scheduling in September.
